Analysis of Ultrasonographic Findings of Idopathic Hypertrophic Pyloric Stenosis and Its Application to Pyloromyotomy

Abstract

|
|
idiopathic hypertrophic pyloric stenosis(IHPS) is an infrequent disease of infants, however it's the most common intraabdominal condition requiring surgery during the first few months of life.
IHPS occurs in the newborn infant, predominantly in males. Pyloromyotomy has been regarded as the treatment of choice, but there is no precise description in literatures about the depth and length of myotomy. we performed the ultrasonographic
evaluation
and the Fredet-Ramstedt's method of pyloromyotomy in twenty one infants with IHPS from september 9, 1993 to August 31, 1995. The sex ratio was 4 to 1(M:F). The mean thickness of stenotic portion was 5.63mm(range: 4.1 to 7.1mm) and the length of
stenotic
canal was 18.55mm(range: 16.2 to 21.3mm). We had made pyloromyotomy at least 2 centimeters in length and to mucosa in depth. No post-operative complications were found. We concluded that the thickness of stenotic portion, more than 4mm, was
compatible
with the diagnosis of IHPS and that the ultrasonographic evaluation should be performed preoperatively. We suggest that two centimeters of length is appropriate in doing pyloromyotomy.
